How DNAK Healthcare Services collects, uses, and protects information submitted through this website.

DNAK Healthcare Services LLC values your privacy. This policy explains what happens to information you provide through this website — what we collect, who processes it on our behalf, and how you can reach us about it.
This is not our Notice of Privacy Practices
This is a website privacy policy. It covers visitors to dnakhealthcare.com.
If you are a client of DNAK Healthcare Services, your health information is separately protected under the Health Insurance Portability and Accountability Act (HIPAA), and your rights are described in our Notice of Privacy Practices, which is a different document. Ask us for a copy at any time — call 470-941-9985 or email info@dnakhealthcare.com.
Information we collect
When you browse this site. This website sets no cookies, runs no analytics, and contains no advertising or tracking scripts. We do not build a profile of you, and we do not know who you are when you read these pages.
Our hosting provider records standard technical information for every request — your IP address, the page requested, the time, and your browser type — in order to serve the site and protect it from abuse. We do not use these records to identify individual visitors.
When you submit a form. The Careers, Set an Appointment, Need Help and Send Your Referrals pages use forms hosted by Google. When you open one of those pages, the form loads from Google’s servers, and Google may set its own cookies in your browser as part of delivering it. That is Google’s processing, governed by Google’s privacy policy. We have configured these forms so that you do not need a Google account to complete them, and so that your browser sends Google no information about which page you came from.
What we collect through a form is whatever you choose to type into it — typically a name, a phone number, an email address, and a description of the care you or your loved one needs.
When you contact us directly. If you call or email us, we keep what you tell us so that we can help you.
Protected health information
Two of our forms — Send Your Referrals and Need Help? — can receive protected health information. A discharge planner describing a prospective client’s condition, or a family member explaining a relative’s needs, is sharing PHI.
Those forms and the accounts that receive them run on Google Workspace under a Business Associate Agreement with Google, as HIPAA requires of a covered entity. Responses are stored in our organisation’s own Google Drive, restricted to authorised DNAK staff.
Please do not send health information by ordinary email. Email is not an encrypted channel end to end, and we cannot guarantee its confidentiality in transit. If you need to share clinical details, use the Send Your Referrals or Need Help form, or call us at 470-941-9985 and we will take the information over the phone.
Who processes information for us
We do not sell your information, and we do not share it for advertising. We rely on a small number of service providers to operate:

We may also disclose information where the law requires it — for example, in response to a court order, a lawful government request, or a state licensing authority acting within its jurisdiction.
How we use what you tell us
- To respond to your enquiry, referral, application, or appointment request
- To assess whether we can provide the care you need, and to arrange it
- To meet our obligations as a licensed home care provider in Georgia
We do not send marketing email to people who contact us through this website.
How long we keep it
We keep enquiries and referrals for as long as needed to respond and to meet our record-keeping obligations as a licensed provider. Client records are retained under the schedule set by Georgia law and our HIPAA policies, which the Notice of Privacy Practices describes. Job applications are retained for a reasonable period in case a suitable role arises, unless you ask us to delete yours.
Your choices
- Ask what we hold. Contact us and we will tell you what information we have about you.
- Correct it. If something is wrong, tell us and we will fix it.
- Ask us to delete it. We will, unless we are required to keep it as a licensed provider or under HIPAA.
- Block cookies. Since this site sets none, blocking cookies in your browser will not affect it. It may affect the embedded forms, which are delivered by Google.
If you are a DNAK client, your rights over your health record are broader than the above, and are set out in our Notice of Privacy Practices.
Security
We use reasonable administrative, technical, and physical safeguards to protect information from unauthorised access, disclosure, alteration, and destruction. This site is served only over an encrypted HTTPS connection. Form responses are held in access-restricted storage under our Business Associate Agreement with Google.
No method of transmission or storage is completely secure, and we cannot guarantee absolute security.
Children
This website is intended for adults arranging care for themselves or a family member. We do not knowingly collect information from children under 13 through this site.
Changes to this policy
We may update this policy to reflect changes in our practices or in the law. Any update will be posted here with a revised “Last updated” date.
